The disks I received were part of a larger set of floppies, and these weren't originals- I've scanned in the hand-written labels on the 5.25'' floppies. I've also included the full file listings generated when I dumped these floppies.

In a box of items I received from a house cleanout, there was this Ace Hardware charge card application in there. The store location is on the back. The physical item is a tri fold brochure- I've tried to arrange the images so they are as you would see them in person. I'm dating this to the 1980s by the expiration date of the cover image, which is 1988- I'm putting 1988 as the latest date this would likely be.
